Student Services' Customer Satisfaction Survey  Part A: Anonymous Feedback

 

Thank you for taking part in this important survey. 

We are asking your thoughts and opinions in order to continue to improve NIC’s Student Services experience. This survey should only take about 5 minutes to complete. Be assured that all answers you provide will be kept in the strictest confidentiality.

There are two parts to the survey. The first, Part A, is confidential and there are no questions that identify you. Responses are aggregated. The second, Part B, is also confidential but allows you to request to be contacted by a member of Student Services to answer a question or address a concern.

Whether or not you answer Part B, you will have the chance to enter your contact information on the prize-entry form to win one of three monthly tuition credit prizes of $150. This entry form is not be linked to your responses.
